The bottom column of the table is filled in only by legal entities in accordance with the constituent documents. The accuracy of the information provided is verified using the data contained in the Unified State Register of Legal Entities and the Unified State Register of Individual Entrepreneurs. If there is a difference between the information contained in the specified registers and the information contained in the documents of a legal entity or individual entrepreneur, the information published in the relevant register will be used as reliable.

It is necessary to indicate both the digital code of the corresponding type of activity and its alphabetic decoding. Indication of unregistered activity codes in the Card will result in refusal of special registration. It is necessary to indicate all types of activities directly related to activities with precious metals and precious stones. However, at present there is no closed list of codes for types of economic activity, in the absence of which special registration is impossible. In connection with the above, for special registration it is possible to use codes of types of economic activities that are not directly related to the use of precious metals and precious stones, but do not exclude their use in the process of carrying out activities, in particular 26.13 - production of glass products. In such cases, it is recommended to accompany the decoding of the codes of types of economic activities with explanations in brackets describing the essence of the transactions carried out with precious metals and precious stones.
General recommendations
It is allowed to fill out a special accounting card for organizations and individual entrepreneurs carrying out transactions with precious metals and precious stones (hereinafter referred to as the Card), both by hand and using specialized technical means. When filling out the Card by hand, it is necessary to ensure that it can be read and that there are no corrections in the Card. When filling out the card handwritten, it is recommended to use blue or black ink. It is not allowed to fill out the “About” section by the applicant himself.
Completing Section 2 Maps
Section 2 of the Map is a table of the form:
In accordance with Article 54 of the Civil Code of the Russian Federation, the location of a legal entity is its registration address. Thus, section 2 of the Map indicates the address of state registration of a legal entity or individual entrepreneur. The “index” column is filled in with numbers from 0 to 9 in accordance with the assigned value. The directory of postal codes is available, in particular, at this link: https://www.pochta.ru/post-index. The column “code of the subject of the federation” is filled in in accordance with the list of subjects of the federation according to the text of Article 71 of the Constitution of the RSFSR of 1978 as amended on November 10, 1992. The Republic of Crimea and the federal city of Sevastopol, which are not included in the list, have numbers 91 and 92, respectively. Columns of the Map containing a note must be filled out in accordance with this note. Thus, for example, in the column “house/possession” it is not enough to indicate “3”; it is necessary to indicate “house 3”, etc. The columns “fax”, “e-mail address” and “official website on the Internet” are filled in only if available. The column “Name of the state Assay Supervision Inspectorate in which the legal entity/individual entrepreneur has/has passed state registration” is filled in in accordance with the areas of activity of the state Assay Supervision Inspectorates. The indication that is subject to the indication is not the state Assay Supervision Inspectorate to which you apply for special registration, but the one whose area of activity coincides with the place of state registration of a legal entity or individual entrepreneur. Filling out the Card header

When filling it out, the person registering for the first time for special registration puts a sign in the square located opposite the value “primary”. When filling out the Card to make changes to previously stated information, you must place a sign in the square located opposite the “changed” value. It is prohibited to fill both squares in one Card. Any sign can be used to fill the square, in particular: “x”, “v”, etc.
